Basic Health Science (BHS) Form Directions

Submission Timing

Do not submit your Basic Health Science (BHS) form before November 1, 2025.
All steps outlined below must be completed before submitting your form on or after this date.


Purpose of the BHS Form

The BHS form (Application Item #4) is used to assess your academic and professional qualifications to determine eligibility for the Shadowing/Interview.

  • Your evaluation, and overall metrics can be updated during the application cycle. To update your BHS GPA with new transcripts: all transcripts must be in your application no later than July 1, 2026.
  • The interview must be completed before finalizing your Baylor/USAGPAN or external agency application (Application Requirement #1).
  • This is the only interview for USAGPAN and is required to compete for admission.
  • Note for 2027 Cohort: Meeting minimum qualifications does not guarantee an interview. Applicants who are not interviewed will not be eligible for admission.

Interview Scheduling

If approved to proceed to an interview:

  • Direct Accession (DA) Applicants (civilians, National Guard, Reservists):
    Must have a Medical Recruiter schedule the interview, contingent on their support and approval.
  • Active Duty (AD) Applicants:
    Will receive interview scheduling instructions from MAJ Kathryn Houhoulis.

Application Caution

IMPORTANT:  Once opened, please do NOT “submit” your Baylor/USAGPAN application early. You may work on the application but must wait for a review process and further guidance before submitting.

  • Early submission offers no advantage.
  • Once submitted, errors cannot be corrected and may negatively affect your application.
  • Additional guidance will be provided.

Step 1: Open Your Baylor/USAGPAN Application

If you’ve already opened your application and selected Anesthesia Nursing (USAGPAN) as your program, proceed to Step 2.

Otherwise:

  1. Open the application before ordering transcripts.
    Transcripts sent without an open application will delay processing.
  2. How to open the application:
    • Use the “Apply Now” button (available November 1) on the Baylor/USAGPAN 2027 Cohort Application Requirements webpage.
    • Complete the short form.
    • After submission, follow the link to the directions for Application Selection (note: new application portal interface for 2027).
  3. For returning applicants:
    • Previously submitted official transcripts and GRE reports will auto-populate.
  4. Need help with login?
    • Call the Baylor Help Desk: 254-710-4357 (CT business hours).

Summary of Step 1:
a. Open the Baylor/USAGPAN application
b. Follow all steps from the Apply Now process
c. Then order official transcripts
d. Proceed to Step 2


Step 2: Complete the BHS Form

Please enter every course you have already attempted or completed that aligns with the subjects below. The names of your courses may differ from the course names provided. Courses or degrees completed after submitting the BHS form will be added to your form only if the transcripts are in the application by July 1, 2026. Please send an email explaining the requested update and providing the institution's name(s) for your new transcript(s).

  • Include all attempts, regardless of circumstances.
  • Include any lab courses completed for items i–xi.

Chemistry Course Requirement: At least one of the Chemistry courses  (in bold font below) is required for  calculation of the BHS GPA as well as completing the documentation of applicant Metrics. The name of your completed Chemistry course may differ from the names listed below.

Important: your nursing degree and any Chemistry course transcript(s) must be from a "regionally accredited." college or university. 

  • Ensure an official transcript is in the application with one of the Chemistry courses listed below, as early as possible in the application cycle/to permit calculation of your BHS GPA and full Metrics review.
  • See the Course List hyperlinked in application item #13 for options.  
  • If you have previously completed a Biochemistry course OR a separate course in Organic Chemistry course then review the information at the top of the Course List to see if it may meet the required standards, submit requests for review to Baylor/USAGPAN Program Coordinator, Jana_Johns@Baylor.edu.
  • The higher the level of difficulty for any Chemistry course taken the more valuable it will be to your application.  Ex. General Chemistry I (for science majors) is typically the highest level of a first college course in Chemistry vs. the lower, less difficult level of a General Chemistry course entitled, Essentials of Chemistry for Nurses or Introductory General Chemistry, etc.
    • The Course List also contains the preferred/higher level of most of the pre-nursing science courses as options for you to consider or choose any one of these courses from any regionally accredited school of your choice.

ii. Anatomy & Physiology II
iii. General Biology I
iv. General Biology II
v. Biochemistry (highly recommended)
vi. General Chemistry I
vii. General Chemistry II
viii. Microbiology
ix. Organic Chemistry
x. Pathophysiology
xi. Pharmacology
xii. Statistics*

*The Statistics course is a requirement for the application but is not included in the Basic Health Science GPA calculation. 

Required Information for Each Course

  • Year: Four-digit year (e.g., 2020)
  • Term: As listed on transcript (e.g., Spring, Summer, or start date in MM/DD/YY format)
  • Department: Course department (e.g., NURS)
  • Course Number
  • Course Title: Use exact name from transcript
  • Institution: Credit-granting institution only
  • System: “Q” for quarter-based or “S” for semester-based (check transcript key or registrar)
  • Grade: As listed on transcript, including plus/minus
  • Credits: Number of credit hours
  • Notes: Add clarifying notes or questions as needed

BHS Needed Documents: 

Preferred: If applicable and completed, submit the 3 Needed Documents listed below in PDF format, in one email or upload them to your application as noted below. 

Document Naming Convention

Use: LastName_FirstName DocumentName

Needed Documents (PDF format required)

  1. REQUIRED: Curriculum Vitae/Resume
    1. Preferably this item is initially sent by email, and once final edits are approved, then upload the approved version to your application, in the Anesthesia Nursing section.
    2. Applicants must follow formatting and content guidance on the Baylor/USAGPAN 2027 Cohort Application Requirements webpage. (See  application requirements #6, #8 and related footnotes).
  2. CCRN or CEN Certificate (if applicable and completed)
    1. You may attach this to your email or upload it into the Anesthesia Nursing section of the application. Your certificate must include the expiration date.
  3. GRE Report (This is an optional item for the application). If completed:
  • You may attach a copy of your Test Taker Score Report to the email or as required in the application, order an Official Institute Score Report sent by ETS to Baylor University, Waco, Texas (CEEB code: 6032).
  • Include all attempts.
  • Do not enter your scores inside the application. 

Submission Instructions

Complete and Submit your BHS form (link below). A confirmation of your submission will automatically be emailed to you (from no_reply@Baylor.edu).

Provide the Needed Documents in PDF format, (as directed in the above section). .

Upon USAGPAN completing a review of your BHS Form and submitted document(s) you will receive a reply email with your initial BHS Metrics, including BHS G)PA, your status for a potential interview, and guidance to improve your competitiveness.

EFFECTIVE STARTING 15 APRIL 2026 

 Do not  submit a Basic Health Science (BHS) Form until you have completed your first meeting with your assigned Medical Recruiter. Additionally, please continue working on your Baylor/USAGPAN application. A meeting with a Medical Recruiter may take 2+ weeks, and the Army Active Duty application, due in June, will take 3+ months to complete. See application requirement #1 for the linked form to "Request a Medical Recruiter."
