The Basic Health Science (BHS) form is now closed.
Updated Information as of 10 AUG 2026
For the 2028 Cohort Application Cycle (applying during 01 NOV 2026 – 01 AUG 2027 and matriculating in JUN 2028).
All steps outlined in the Basic Health Science (BHS) Form Directions must be completed prior to submitting your BHS form.
***ACTIVE DUTY NURSES ONLY***
01 OCT – 31 OCT – Early submission of the BHS form is permitted for AD Nurses only, with email submission of unofficial or student copies of transcripts. Please use your Army email address and follow all the BHS form directions prior to submission. Do not order official transcripts until after you have opened your Baylor/USAGPAN application on/after 01 NOV.
ALL OTHER APPLICANTS (Civilian, National Guard and Reserve):
WAIT to submit until 01 NOV and ensure you follow all the directions provided for the BHS form prior to submitting it. Early submission is not permitted. Any submission prior to 01 NOV will not be processed, and a new BHS form submitted on/after 01 NOV will be required. Do not order official transcripts until after you have opened your Baylor/USAGPAN application on/after 01 NOV.

The Baylor/USAGPAN application cycle opens on 01 NOV and closes on 01 AUG, annually, with additional deadlines preceding 01 AUG.
