Basic Health Science Form

***IMPORTANT*** Application Requirements, all directions and policies, etc. are subject to change for the next and future application cycles, the 2028 Cohort application cycle opens 01 NOV 2026. 

The Basic Health Science (BHS) form is now closed.

The BHS form is open from 01 OCT* - 01 JUN during each application cycle. Please return to submit the BHS form by closely following the Directions for BHS Form. 

* Submissions between 01 OCT and 31 OCT ONLY, may email unofficial transcripts, starting 01 NOV no unofficial transcripts/follow the Directions for BHS Form.

The Baylor/USAGPAN application cycle opens on 01 NOV and closes on 01 AUG, annually, with additional deadlines preceding 01 AUG.
