Request Medical Recruiter

This form is required for all Civilian, National Guard and Reserve status applicants to Baylor University’s U.S. Army Graduate Program in Anesthesia Nursing (USAGPAN). It is highly recommended that applicants request a Medical Recruiter in late summer or fall preceding the year they plan to submit the Army Active Duty and Baylor’s USAGPAN applications.

For example, the 2027 Cohort Baylor/USAGPAN applicants should request a Medical Recruiter the summer or fall of 2025, prior to the opening of the 2027 Cohort application on Nov. 1, 2025, and well before the Army application is due in June 2026.

In order to complete this form, you must confirm the following:

• That you are a U.S. Citizen. U.S. Citizenship is required at the time of this request, no exceptions. Permanent Residents are not eligible. The Medical Recruiter will require documentation of your citizenship.

• That your highest nursing degree is from a U.S.-based and U.S.-accredited nursing program.
