This form is only active from 01 OCT - 01 MAY during each application cycle. The Medical Recruiter (a specialized recruiter for Nurses and other medical professionals) facilitates a 3+ month process to guide you through the external application, the Army Active Duty application, which is due by early June in each application cycle (see Baylor/USAGPAN application requirement #1).

Please return on/after 01 OCT to submit the form, Request a Medical Recruiter, for the next available application cycle. The Baylor/USAGPAN application cycle opens on 01 NOV annually.
